Accountable Care Organizations (ACO) and Bundled Payment Program Participants

Legislation. Regulations. Constant changes. It’s all had a dramatic impact on how healthcare providers deliver care. The introduction of ACOs and bundled payment initiatives such as BPCI (Bundled Payments for Care Improvement) and CJR (Comprehensive Care for Joint Replacement) seem to now be the drivers of cost transparency and care coordination across the post-acute continuum.

The way FOX is structured helps you offer clinically excellent care at a cost less than many other therapy alternatives. FOX Geriatric House Calls™ bridge the gap between Traditional Medicare Part A Home Health and Part B Outpatient Care. That allows FOX to offer better outpatient therapy compliance rates, while delivering care to patients in the comfort of their home.

Hospitals and Outpatient Rehabilitation Facilities

A Post-Acute Care Continuum starts when the patient is discharged from an acute care facility (i.e., multi-disciplinary hospital or stand-alone ambulatory surgery center). It usually ends with a prescription to finish therapy at an outpatient rehab facility—often a part of the same health system where the patient received acute care. FOX Rehabilitation partners with these health systems to improve compliance with prescribed outpatient therapy. Some patients may not qualify for home health services under the medically homebound criteria. On the other hand, barriers may make them unable or unwilling to leave their homes for the care they need to avoid re-hospitalization. FOX can help. We strengthen patients in their own homes until they are ready mentally and physically to continue care in an outpatient clinic setting.

Skilled Nursing Facilities (SNF)

Reduce your patient length of stay and reduce re-admissions to hospitals. This is the paradox every SNF director faces. Both ACOs and bundle programs encourage the creation of preferred-provider or high-performance networks. These can leave SNFs with reduced referral patterns. Partnering with FOX can reduce re-hospitalizations because we stay with patients after they are discharged. Whether they’re leaving the SNF (when patients don’t qualify for Home Health) or post-Home Health when a dual-script continuity of care is used. A FOX partnership can also improve statistics that matter to upstream referral providers like hospitals. We help reduce re-hospitalization rates and can potentially improve the SNF census since FOX clinicians are trained to recognize a re-hospitalization risk. They immediately recommend that the patient be re-admitted to the SNF, rather than ending up in a hospital emergency room.


Home Health Agencies

Home Health Agencies are often short of qualified clinicians to meet the needs of Medicare Part A patients. The homebound restriction also limits the amount and duration of care they can provide under current reimbursement structures. FOX Rehabilitation has many successful partnerships with home health agencies, both as a sub-contracted provider of therapists, and as a clinical partner that can improve function, monitor the patient, and improve quality of life post Home Health discharge.
